085 MARTIAL ARTS SUMMER CAMP 2026
REGISTRATION FORM - STARTING JUNE 15, 2026
A premium martial arts and leadership summer experience for children ages 5-12.
Camp hours: 9:00 AM - 4:30 PM.
Extended care/kids class availability until 6:00 PM.
Late pick-up fee applies after 4:30 PM (except for students) : $1 per minute per
child
PROGRAM DETAILS, MEDICAL INFORMATION & PAYMENT
Activities may include Brazilian Jiu-Jitsu, Muay Thai, self-defense, bully-proof leadership, team games,
sports, mini golf at The Grove, District Park sports activities, Mirada Lagoon activity days, outdoor
recreation, and transportation using the official 085 van. Camp students will join the official 085 Homeschool
Martial Arts class every Monday, Wednesday, and Friday at 10:00 AM.
TUITION & FEES:
085 Members:$239 per week
Non-Members:$259 per week
Registration Fee: $99 per child - non-refundable
Late Pick-Up Fee: $1 per minute per child after 4:30 PM
PAYMENT INFORMATION:Card is the only accepted payment method. A valid card must remain on file at all times. Billing information will be completed in-office through our secure system.
LUNCH, SNACKS & WATER: Parents are responsible for providing lunch, snacks, and a labeled water bottle each day unless otherwise
announced in writing by 085 Martial Arts. Please do not send food that conflicts with allergy restrictions
announced by the camp.
TRANSPORTATION & FIELD TRIPS AUTHORIZATION:
I authorize my child to be transported in the official 085 Martial Arts van for approved activities and field
trips, including but not limited to The Grove, District Park, Mirada Lagoon, and other scheduled camp
activities. Students will remain supervised by staff during transportation and activities.
CAMP POLICIES
Students must follow coach and staff instructions at all times. Camp shirts are required for field trips. Only
authorized adults may pick up students and ID may be required. Respectful behavior is mandatory. Bullying,
unsafe behavior, or repeated disruption may result in dismissal without refund.
LATE PICK-UP POLICY
Standard camp pick-up is by 4:30 PM. A late pick-up fee of $1 per minute per child applies after 4:30 PM.
Students staying for evening kids classes must be picked up immediately after class ends at 6:00 PM. Late fees
still apply after 6:00 PM.
PAYMENT POLICY
Card is the only accepted payment method. Weekly tuition and applicable fees will be charged to the card on
file. A valid card must remain on file at all times. Cash, Zelle, check, and other payment methods are not
accepted for Summer Camp registration or tuition.
NO REFUND POLICY
All registrations are non-refundable and non-transferable. Payment is required for all reserved weeks regardless
of attendance. Missed days are not credited or refunded.
LIABILITY WAIVER & ASSUMPTION OF RISK
I understand that martial arts training, sports, transportation, field trips, outdoor activities, and camp
activities involve inherent risks. I voluntarily allow my child to participate and assume all risks. I release
and hold harmless 085 Martial Arts, its owners, instructors, staff, contractors, and affiliates from claims
arising from participation, except where prohibited by law. I confirm my child is physically able to
participate.
PHOTO & VIDEO RELEASE
I authorize 085 Martial Arts to use photos and videos of my child for marketing, social media, website, and
promotional materials.
FINAL AGREEMENT
I understand and agree to all policies above. I confirm that the information provided is accurate and that I am
responsible for all payments, late fees, and camp policies.
